The Sustainable Growth Rate (SGR) payment system was a decade-long concern for physicians receiving Medicare reimbursements. Although the U.S. Congress repeatedly implemented "doc fixes" to patch the broken SGR system, it has finally been replaced by the Merit-Based Incentive Payment System or "MIPS." This new system will combine three quality incentive programs for physicians through Physician Quality Reporting System, Value-Based Payment Modifier, and Meaningful Use of Electronic Health Records. However, there are winners and losers under MIPS. Every physician receives a performance score that can either positively or negatively affect reimbursements.
