Cancellation A $100 service charge will apply to all written cancellations received by Thursday, September 9, 2010. No registration fees will be refunded if cancellation requests are received after Thursday, September 9, 2010. Physicians-in-Training + Medical Students **A limited number of scholarships is available to cover the full cost of registration for physicians-in-training and medical students who intend to attend all three days of the conference. A $300 security deposit and a letter verifying academic enrollment is required at the time of registration. If you register online, please send in the letter via fax at 213-2572-0635, mail at 625 W. College St., Suite 209, Los Angeles, CA 90012 or email at camasc (at) foundationpamc.org. The deposit will be refunded at the conclusion of the conference on-site or via mail. Refunds will not be given to no-shows or part-time attendees. Thursday, September 9, 2010 is the deadline for physicians-in-training and medical students to register or cancel. A full refund will be issued for cancellation requests received in writing via email, fax, or mail by Thursday, September 9, 2010.
